2008 Future Aces Citizenship Award & Scholarship Reception

Upon written confirmation of being chosen as a Future Aces Citizenship Award and Scholarship Recipient, winners will need to provide:
- 5-6 digital photos submitted by email. These will be used for a video presentation on the night of the reception.
- Pictures should include one graduation photo or recent head shot and several shots showing you in activities.
- A signed Release Form giving the Foundation permission to share your story with the media or post on our web site.
- The number of guests attending from your family, school or community.
- The school representative who will be part of the presentation.
Forms will be sent out to each recipient requesting the information above.
